FSSAI registration

Form8 Life Sciences Pvt Ltd is registered under the Food Safety and Standards Authority of India (FSSAI). Our FSSAI registration number is 21226181000106. We manufacture and/or market products in categories permitted under the Food Safety and Standards Act, 2006, and applicable regulations, including labelling, claims, and licensing requirements for foods and nutraceuticals as updated from time to time.

Good Manufacturing Practices (GMP)

Our products are produced at facilities that operate under applicable FSSAI licensing conditions and Good Manufacturing Practices relevant to the product category. We maintain documentation of vendor qualification, incoming material checks, in-process controls, finished-goods release, and traceability by batch and lot number. Specific GMP certificates and audit summaries may be shared with trade customers and regulators under confidentiality where appropriate.

Third-party laboratory testing

Each batch of every product is tested by independent, accredited third-party laboratories for parameters appropriate to the formulation (for example, identity, potency, heavy metals, and microbiological limits as applicable). Certificates of Analysis (“COA”) are available on request — email info@formeight.in with the batch number printed on your label. We do not rely solely on in-house testing for release of finished goods where independent verification is material to safety or label claims.

Not for medical use

Form8 products are foods / dietary supplements, not drugs or medicines. They are not intended to diagnose, treat, cure, or prevent any disease. Label copy, website content, and customer communications avoid drug-like claims and comply with FSSAI guidance on health supplements and nutraceuticals. Customers with medical conditions should consult a physician before use.

Advertising and ASCI

Our marketing follows the Advertising Standards Council of India (“ASCI”) Code and the self-regulatory framework for food and beverage advertising, including nutraceuticals, together with Central Consumer Protection Authority guidelines on endorsements and misleading advertisements where applicable. We do not use misleading before-and-after imagery, suppressed material information, or unsubstantiated superlative claims. Scientific references, where used, are presented proportionately to the evidence and in line with permitted structure/function language for foods.

Pre-launch phase

Phase 1 of Form8 may operate as a pre-launch waitlist or limited programme while licence upgrades or listings are finalised. No commerce is enabled until we confirm that required permissions are in hand. This page will be updated as our regulatory posture evolves; the “Last updated” date reflects the latest general revision.
